My last campaign was a mix of domain and adventure, the one I'm currently running is (so far) character based. It has plenty of domain-level factors as a back-drop, though. So, obvously, I went with the Mix-alternative.

Today, we were kidnapped by hill folk never to be seen again. It was the best day ever.

the game I am planning starts with a pure adventure focus. Through the actions of several NPCs, the PCs are forced into control over a domain. From then on the game will have a domain focus.

when we still played BR, i let the players roleplay the domain turns, so they had to think on how to handle things... and even end up in an adventure because of it, not just rulling the dice.

Not currently running a game in Birthright, but three
folks and myself are planning to start one next month.

We are using the Birthright setting as truly as we
can; but not starting with any characters being
blooded. Love of the setting makes it fun to
    adventure there!

Never know when someone may become blooded in one way
or another! A thief in a game I played in before
kidnapped a blooded scion`s family members and forced
his way to becoming a scion by way of investing the
power of the ruler...can`t remember the Brecht realm
    he took over...

I'm putting in "mix" here, but actually this depends a lot on the campaign...some campaigns focus more on the domain play, while others are 100% adventure style (however, even those games have a clear domain backdrop, only the players are not in charge of any domains).
