IIRC he's old orog. How he can keep STR and CON at 18?
He's 12th level - how he still have INT13? With 3 upgrades to attributes?
And IIRC, his regeneration stems from Ring of Regeneration.
I don't have Rjurik Highlands now, so I can be wrong with last statement.
Highlands say he's STR & CON 18 and INT13. And yes, he has the ring.

'Old' for an orog could still mean prime of his life - a visibly weak leader would be slaughtered.
Per Highlands he also has the regeneration, battlewise, and detect lie abilities - quite a sweet array.
He's designed as a 2e character though, i.e. random rolls, toughest guy wins, not on a 3.5e elite array or suchlike. If you prefer a 'proper' rebuild the wiki is great for alternate versions...

He's also supposed to be a serious problem, an elite enemy, not a run of the mill orog to dispatch.

The BRCS description of Orogs says they are a +3 ECL race. Does that mean that Thrakkazz's CR should be 15 not 12?

+3 ECL means as a PC, Thrakkazz would have to look three levels up on his experience table. To get to level 13, he needs the experience of a 16th level human. There is a separate CR adjustment, but currently the character is missing his extra orog hit dice, special orog abilities, and so on. He's just got the Highlands materials added to his page.
I'll look at a more complete build of this NPC.

ECL is only for players and is never figured into the CR calculations.
