I would say the fortifications would still be there at it former level
in both cases, similar to a castle. After a fortified holding represents
a large building that has been fortified. Just because your holding has
been contested or the province level lowered does not mean that the
building has changed.
In my game I allow fortified holdings to be reduced, but they can not be
reduced below level 0 unless besieged and destoryed.

Are the fortifications "reduced" by drops in province levels (magic,
pillaging, levies ...) How are the fortifications restored to their former
level? (ie successful rule or opponent's failed contest action)

No, fortifications are not lowered when a province drops in level. The
holding level is, but not the fortifiaction.
